Your first appointment to our office includes an oral evaluation and a consultation. We’ll get to know you, help you understand your diagnosis, and discuss your options for treatment.

It is important that you bring the following items with you to your first visit:

New patient paperwork that was emailed to you when setting up your appointment. If you didn't receive the paperwork, please click here to print it out and bring with you. **If you choose to fill out the paperwork in the office, please arrive 20 minutes prior to your scheduled appointment to allow time to complete this information.**

Referral slip from your dentist

Any applicable X-rays and list of current medications

Medical and/or dental insurance information

Driver’s license or another government-issued ID

If you are under 18 years of age, a parent or guardian is required to attend the consultation visit with you.
